As a Dungeon Master there are few things more enjoyable than tossing your players a few cursed items. The 5e core rule books actually have some fun cursed items built right in, but why stop there? There are so many things you can do! The items that are cursed are just mixed right in with other magical objects. They are labeled with a small Curse heading, indicating that the item is in fact cursed and does something fun.
As we talked about in our previous Cursed Items article, there is no special guidelines for making cursed items. The best ones are the ones your players want to use. Balance the items based on what they do. If you have a ring that increases an ability score, have it reduce a different one.
If you grant resistance to one type of damage, add vulnerability to another. If the items gives you any type of benefit in one regard, you can negate it in another. This is something we see in the pre-made items, particularly the Armor of Vulnerability and the Shield of Missile Attraction. You can adjust the severity of the downside or benefit of any item to make using it more or less punishing for your players.
Most cursed objects in DnD affect combat, but consider giving your players non-combat cursed items. The trick here is to work items into the story or role play of the game. While it is hilarious to give a player a heavy rock that is always in their inventory, it just makes the players have to do more math and is not super fun or relevant to the game.
As an example, you can make a ring of gluttony that makes someone devour twice as many rations. Making magical items and curses is one of the most fun parts of DnD for the Dungeon master. This guide puts together simple step by step instructions for creating cursed items on the fly. The cursed object could be anything. You could choose a weaponcustom itemor even balance out a magical weapon. You can even pick mundane items if you like.
If something is going to be cursed, players need a reason to interact with it. While there are a million different ways you can curse an object, the list below covers most of your basic options and can be applied pretty much anything. If a curse requires specific item types it will be noted in different sections. Curses can last for a long time, a short time, or forever. When you choose a duration of condition for the curse to be applied, try to be fair with it.
Players might not know a curse is removed, so keep it to yourself in case you need to make it easier to remove later.Mdcn november 2019
While this may be a bit of a trap for players as it grabs their curiosityit also makes sense that a cursed object would want to be noticed and in its own way demand a character see it. Just describe the object plainly and in detail. When using cursed objects, be careful.
5e Cursed Items

Occasionally they mix bad with good, forcing characters to make difficult choices. The user believes the item is what it appears to be, yet it actually has no magical power other than to deceive. The user is mentally fooled into thinking the item is functioning and cannot be convinced otherwise without the help of a remove curse spell.
These cursed items malfunction, so that either they do the opposite of what the creator intended, or they target the user instead of someone else. Opposite-effect items include weapons that impose penalties on attack and damage rolls rather than bonuses.
Once she knows, however, the item can be discarded unless some sort of compulsion is placed upon it that compels the wielder to keep and use it. In such cases, a remove curse spell is generally needed to get rid of the item. The three varieties of intermittent functioning items all function perfectly as described—at least some of the time.
The three varieties are unreliable, dependent, and uncontrolled items. The item only functions in certain situations. To determine what the situation is, either select a situation or roll on the following table.
An uncontrolled item occasionally activates at random times. On a result of the item activates at some random point during that day.
In a sense, a command word is a requirement. Nevertheless, some items have much more stringent requirements that must be met for them to be usable.
To keep an item with this kind of curse functioning, one or more of the following conditions must be met. Requirements are so dependent upon suitability to the item that they should never be determined randomly. An item with a requirement that is also intelligent often imposes its requirement through its personality.
If the requirement is not met, the item ceases to function. If it is met, usually the item functions for one day before the requirement must be met again although some requirements are one time only, others monthly, and still others continuous. Items with drawbacks are usually still beneficial to the possessor but they also carry some negative aspect.
Although sometimes drawbacks occur only when the item is used or held, in the case of some items such as weaponsusually the drawback remains with the character for as long as she has the item. Specific Cursed Items are provided as examples of cursed items. They are given creation prerequisites, should someone want to intentionally create them although that does not need to be the origin of the item.
Note, however, two exceptions: The crystal hypnosis ball and the bag of devouring cannot be created by any known means. A simple detect magic spell yields a misleading aura and strength, often indicating that the item is a noncursed item of similar sort.This title is a collection of magic items with powerful effects and insidious curses. In this article, we will explore official rules and non-canonical ideas for crafting magic items, weapons, and potions.
We will also provide a table of examples at the end of the article of crafting requirements for homebrew magic items to players and Dungeon Masters alike. According to pages and of the Dungeon Masters Guide, a Dungeon Master can allow a player to craft magic items, though it is an expensive and lengthy task. Remember: all aspects of crafting magic items are under the purview of the Dungeon Master, who has final say on rules. A player crafting a magic item makes progress in increments of spending 25 gp of the Creation Cost per day, assuming they work for 8 hours each of those days.
This includes having or expending if required by the spell material components once each day, unless the item has a one-time use like a spell scrollin which case, the components are only consumed once. Finding components can make for a fun side quest.
Multiple Contributors. If multiple characters meet the level requirements, they can work together to create a magic item. Each character can contribute spells, spell slots, and components, and all characters must participate during the entire crafting process each crafting day.
Each character can contribute 25gp worth of effort per day crafting the item. This means that the time required can be divided by the number of contributing characters. Note that the DMG does not say each character must have spell slots or the spell to contribute. Only one of the characters needs the spell requirements.
Thus, any character can contribute to creating the magic item if they meet the level prerequisites. Example: Four characters crafting a Very Rare item can create it in days instead of 2, Developing formulas for a magic item is less complicated based on DMG rules, as usually only a spell and associated components are required unless the DM decided otherwise.

There are random tables for just about everything, but I did not see one for curse effects. But I was hoping for something more official. Also I imagine two types of tables.
A curse that has one continuous effect and a curse that has a sort of random effect every day or week. Since these types of effects tend to be more edition agnostic than other rules, you might want to consider earlier sources. Dragon Magazine would often run an article on new ways to curse a player. The Hex Spell PHB pg not a lot of options there though If you use the scroll and potion mishaps, you will want to remove the beneficial effects. There is a list of Vistani Curses on page 28 of Curse of Strahd.
